The Housing and Support Partnership can work with organisations and service users to maximise the uptake of benefits and funding streams such as the following:

• Profile and maximising income – HB, SP grant, IS, ILF, etc
• Applications for Disabled Facilities Grant or Social Fund
• ISMI Income Support for Mortgage Interest
• Formal applications – for care assessment, social housing, mortgage
• Problem solving
• Recruiting relevant expertise
• Managing the individual plan
• Monitor programme and progress
• Organising help with moving in
• Making best use of personal funding/family capital

This work can be carried out alongside the Housing Option Planning process to help implement individual plans or it can be undertaken as a discreet project.
